Two people both feel tired all the time. In Western medicine, they might receive the same diagnosis — chronic fatigue, or simply "stress." In TCM, they could be described with entirely different patterns. One might be "qi deficiency" (气虚): pale complexion, short of breath, soft voice. The other might be "liver qi stagnation" (肝郁): irritable, chest tightness, frequent sighing. Both are tired, but the TCM description points to different functional states — and suggests different approaches. This is why a formula is matched to a pattern, not directly to a symptom. Two people with the same Western condition may use different formulas; two people with different Western conditions may use the same formula if their patterns align. When reading a PCMWiki entry, the patterns field at the top of the page tells you which description the formula was traditionally created for — and that context matters more than the ingredient list alone.
